Doctor's Visit

An individual visits a physician an average of 3 times per year.(1) The most common office visit CPT codes are:
99214 = $85, (Office Visit - High Severity)
99213 = $45, (Office Visit - Moderate Severity)
99212 = $31.88 (Office Visit - Minor Severity)

Plan provides for physician office visits that exceed the national average. PPO network can be accessed to reduce price

Diagnostic Test

Over 18% of population visit a physician each year for preventative care. A physician will require a diagnostic test (x-ray, lab work, etc.) more than 87% of the time.(1)

Plan provides a wellness benefit with annual diagnostic testing.

Prescribed Drug

A drug is prescribed after a physician visit over 73% of the time.(2) More than 66% of the population will use a prescription drug in the next year.(6) Average generic brands cost $21 and are prescribed 65% of the time as brand cost $126 and are prescribed 35%(8)

Plan includes prescription discounts:

Hospital Stay

6% of population spend 1 night in hospital and less than 1% spend a night in the hospital on two separate occasions.(3) Average retail cost is $1552; with PPO network, cost is $1352.(5)

Plan includes hospital benefit with 1st time occurrence rider for calendar year to meet 99% of needs.

Minor Surgery

31% of population will visit an outpatient facility(4) with 6.2% requiring minor surgery.(7)

Plan includes surgery and related anesthesia benefit to be applied to surgeries. Also can be discounted within PPO network.

Accident

9.3% of population will have an accident or poisoning related event requiring medical treatment.(3)

Plan includes accident protection to cover most accident events.
