Your new insurance comes with a powerful accident and injury coverage.

It will cover all of your accident or injury medical bills up to the amount shown below (after $100 deductible) .

You can use your insurance for any treatment medically necessary by a physician or dentist as a result of an accident.
Examples: hospital room and board; dental work to sound natural teeth; drugs; medicines; diagnostic tests and x-rays; treatment performed by licensed medical professionals; or rental of durable medical equipment.

• Bronze Plan: Covers up to $2,500
• Silver Plan: Covers up to $5,000
• Gold Plan: Covers up to $7,500

How do you use it?

At the time of your accident you contact your representative via phone number that you receive in your packet.
Once your claim is filed you pay a $100 deductible.
Your insurance will then cover up to the amount listed based on the Plan you choose.
